When families start touring assisted living communities, they often leave feeling encouraged โ the spaces are beautiful, the staff is warm, and the quoted monthly rate feels manageable. Then the contract arrives, and the real cost comes into focus.

The Base Rate Is Just the Starting Point
Every assisted living community quotes a base rate โ typically the cost of an apartment with standard amenities and a basic level of personal care. In 2026, national base rates average around $4,500โ$5,000 per month. But for most residents, the actual monthly bill is considerably higher.
Here's what gets added on top of the base rate:
Care Level Fees
This is the biggest variable most families don't anticipate. Assisted living communities assess each resident's care needs and assign a care level โ typically Level 1 through Level 4 or 5 โ each with an additional monthly fee.
A resident who needs help with bathing, dressing, and medication management might be assessed at Level 2 or 3, adding $400โ$900 per month to the base rate. A resident who needs two-person transfers, incontinence care, and behavioral support could be assessed at Level 4 or higher, adding $1,200โ$2,000 per month.
When you tour a community, always ask: Based on my loved one's current needs, what care level would they likely be assessed at, and what does that add to the monthly cost?
Move-In and Community Fees
Most assisted living communities charge a one-time community fee โ often called an administrative fee or move-in fee โ ranging from $1,500 to $5,000. This fee is typically non-refundable. Some communities also require a security deposit.
Ancillary Services
Services that are not included in the base rate or care level fees include:
- Transportation to medical appointments ($15โ$40 per trip)
- Personal laundry (if not included in the care level)
- Beauty salon / barber services
- Physical, occupational, or speech therapy (billed separately through Medicare or private pay)
- Guest meals
- Incontinence supplies (some communities include these; many charge separately)
Annual Rate Increases
Assisted living rates increase annually โ typically 3โ6% per year. A community that costs $5,000 per month today may cost $5,800โ$6,300 per month in three years. When evaluating affordability, always project costs out 3โ5 years, not just the current rate.

- How is care level determined, and how often is it reassessed?
- What triggers a care level increase, and how much notice will we receive?
- What is the community's average annual rate increase over the past 3 years?
